SEP (Systematic Evaluation Plan)
SEP helps you build and maintain a Systematic Evaluation Plan tied to your accreditation body—linking standards to measurable evidence already in HealthTasks.
Overview
Open Accreditation from the sidebar, then select the SEP tab. Admins create and edit SEPs here. SEP is part of the Intelligence plan’s accreditation toolkit, alongside Self Studies.
Older SEP bookmarks redirect to Accreditation → SEP.
Create a SEP
- Open Accreditation → SEP.
- Create a new SEP with a clear name.
- Choose an accreditation body: CCNE, ACEN, or CNEA.
- Optionally set the current cycle or reporting period label.
The body you select seeds the standards structure for that plan. Open the SEP to work standard by standard.
Link measurements
Inside a SEP, attach evidence to standards and reporting periods by linking:
- Evaluation templates — pull from native evaluation data
- Program outcome items — pool results from evaluations, skills, checkoffs, and assignments
- Manual entries — enter values yourself when evidence lives outside HealthTasks
For program-outcome links, you can choose which data streams feed the result. For native program outcome measurements, you can also restrict cohorts:
- All cohorts (default) — includes every student cohort, matching course-level PO coverage.
- Selected cohorts — limit the measurement to specific cohort codes (the same values set on student profiles in Users).
Use cohort scoping when one SEP tracks multiple program tracks or entry points and you need outcome data separated by class year or campus. Keep links specific so status stays meaningful for site visitors and continuous improvement.
Sync and status
Native-linked measurements sync from live program data when you open the SEP and when you refresh. Typical sync states include synced, no data, pending, or error. Row-level status helps you see what is met, below target, or still pending.
If a measurement shows no data, confirm the linked template or outcome has activity in the selected period and cohort scope.
Export
Download a merged SEP report from the list actions or from SEP detail (Export report). Use exports for accreditation packets, leadership review, or archival copies of a cycle.
You can also duplicate a SEP when starting a new cycle from a prior structure, then update links and narrative fields.
Tips
- Create one SEP per accreditation body or major program track you need to maintain separately.
- Prefer linked measurements over manual entry when the evidence already exists in HealthTasks.
- Review sync status before exporting for a visit or annual report.
- Keep narrative fields (analysis, action plans, frequency) current as you close the loop on findings.
Troubleshooting
SEP tab missing or upgrade prompt
SEP requires the Intelligence plan and admin access. Auditors may see Accreditation Self Studies without the SEP admin tab.
Standards look empty
Confirm the accreditation body was selected at creation so the plan could seed its standards. Open the SEP detail, not only the list.
Measurement stuck on no data or error
Check that the linked evaluation or program outcome has data for the period and filters you chose. Refresh after new activity posts.
Export failed
Retry from SEP detail. If it continues, contact support with the SEP name and accreditation body.
